The title: הלכת שחיטה כ.\ תצניף כג"ק מרנו ורבינ[ו.]\ שמואל הדיין המשכי[ל.]\ זצ"ל.
The title may be referring to a שלמה הלוי בר שמואל הדיין המשכיל, who is the scribe of a manuscript codex of Maimonides' Commentary on the Mishnah (Oxford Bodleian 394/399) dated 1223, and a codex of Mishneh Torah, Book of Mishpatim (ms. Escorial G III 2).
A collection of responsa copied by הדיין המשכיל זצ״ל appears in a booklist in Oxford Bodleian Heb. d. 76 fol. 53.
